When to use this template
Most small teams cannot justify an ATS like Greenhouse or Lever for their first 5-20 hires, but they desperately need a structured application form so candidate evaluation does not become a chaotic email thread. This template covers the data hiring managers actually use — identity, contact, role applied for, experience level, education, LinkedIn and portfolio URLs, earliest start date, and a free-form motivation field. Skip the ATS expense, paste the form on your careers page, and you have a structured, exportable list of applicants by week one. The fields are calibrated to be enough for first-pass triage without scaring off strong candidates with a 30-field intake. Add a file upload for resumes (FormGen Pro) and you have everything you need until your hiring volume justifies a real ATS.

Customization tips
- Add a file upload field for resumes on FormGen Pro plan — most candidates expect to attach one
- Replace the generic Engineering / Design / Marketing options with your actual open roles, and add scheduling so the form auto-closes when a role is filled
- Use conditional logic so engineering candidates see a "What is your strongest stack?" follow-up, designers see a "Portfolio walk-through link" prompt
- Pipe submissions to a Notion or Airtable database that your hiring team can review collaboratively
- Set up auto-rejection emails for clearly-unqualified applications via a webhook + simple rule (matches keywords or experience floor)
Frequently asked questions
When should I switch to a real ATS?
Around 50-100 applications per role, or once you are running 3+ roles at once. Below that, an ATS is overkill — the time saved on workflow is less than the time spent administering the tool. A structured form + a Notion/Airtable database scales surprisingly far.
How do I handle EEO compliance?
For US hiring, EEO data (race, gender, veteran status) must be optional, separated from the rest of the application, and not used in hiring decisions. Add a separate optional EEO survey via a second form, not as fields on the main application. FormGen has scheduling and conditional logic to make this clean.
Should I require a cover letter?
Generally no. Cover letters as a hard requirement reduce application rate by 40-50% and the signal-to-noise ratio is poor — they reward candidates who know how to write a cover letter, not necessarily candidates who can do the job. The optional "Why are you interested?" field captures the same signal at lower friction.
