When to use this template
Course registration is more than a payment transaction — it is the first touchpoint of the learning experience, and the data you collect at enrollment shapes everything that follows. The instructor uses experience-level data to pace early lessons. The marketing team uses the "how did you hear about us?" data to refine ad spend. The student uses the "what do you hope to learn?" answer as a self-clarifying exercise that improves their own outcome. This template captures all three, plus the standard identity and course-selection fields. It works equally well for one-off workshops, multi-week cohort programs, evergreen self-paced courses, and corporate training. Pair it with Stripe checkout for paid courses or use it standalone for free intro workshops that feed a paid back-end.

Customization tips
- Replace the generic Introduction / Fundamentals / Advanced / Masterclass options with your actual course names and dates
- Use scheduling so each cohort's registration form auto-closes when capacity is reached or the start date passes
- Add a Stripe payment integration on Pro plan to collect course fees at registration — dramatically lowers no-show rates vs. invoice-after
- Pipe the learning-goals field to your instructor — reading 30 students' goals before week 1 is one of the highest-leverage things any teacher can do
- For multi-cohort programs, segment the welcome email automation by experience level so beginners and advanced students get different prep materials
Frequently asked questions
How do I handle paid course registration?
Two patterns. (1) Collect payment at registration via FormGen Pro's Stripe integration — best for paid courses where you want to lock in commitment. (2) Send a Stripe Payment Link in the auto-confirmation email and treat the registration as pending until paid — works for high-trust audiences. Pattern 1 has lower no-show rates; pattern 2 captures more leads who pay later.
Should I cap registrations?
For cohort programs, yes — a hard cap (with a waitlist after) creates urgency and protects the instructor experience. For self-paced courses, no cap unless your platform has technical constraints. For workshops, cap at the room or video capacity minus a small buffer for tech issues.
Can I use this for corporate training?
Yes — replace the personal email field with a work-email field and add a "Manager email" field for approval workflows. For larger programs, pipe registrations to your LMS via webhook so completion tracking flows automatically. The scaffold is the same; the customization layer changes.
