When to use this template
A quote request form is the difference between sending a proposal in 24 hours and chasing a prospect through three rounds of clarification email. Without enough upfront detail, you cannot price the work; with too much, prospects bounce before they finish the form. This template is built around the four pieces of data every quote needs: service tier, project scope, budget expectations, and timeline. Everything else is optional. The Budget Range field is doing the most work — it lets you categorize requests instantly into "yes, write a proposal", "let us scope a smaller phase", or "polite decline" without ever picking up the phone. Should I show prices on the form?
For packaged tiers, yes — list approximate ranges next to each option ("Basic, $500-$1,500"). Transparent pricing filters out budget mismatches without insulting anyone. For custom work, leave pricing off the form and lean on the budget-range field to triage instead.
How quickly should I respond to a quote request?
The conversion-rate research is unambiguous: same-day response wins. After 24 hours, the prospect has either moved on or contacted three competitors. If your standard turnaround is longer, set the expectation in the thank-you message ("We send quotes within 2 business days") so prospects do not assume you forgot.
When do I use Request Quote vs Client Intake?
Request Quote is for known scope where the prospect already knows what they want — a 1500 sq ft paint job, a 4-page brochure, a 3-hour photoshoot. Client Intake is for exploratory engagements where scope is itself the first deliverable. Pick the form that matches where the prospect is in their thinking.
